As the parent or guardian of a child being registered to participate in a Junior Jaws/Mini Mermaids Holiday Clinic/Deadly Choices/One Heart/Equal Access Program/Experience, you are acknowledging that the Program/Experience is a sport and recreational activity that may involve your child participating in sporting activities of a physical nature.
By registering for the Program/Experience, you agree that you have assessed the activities that make up the Program/Experience and warrant that your child is physically and mentally capable to participate in the relevant Program/Experience's activities. You also warrant that your child does not suffer from any physical, mental or health condition that could be aggravated, worsened or impaired by their participation in the Program/Experience.
By registering for the Program/Experience, you acknowledge that there are risks of harm or injury to your child as a result of their participation in the Program/Experience (including the risk of serious physical injury or death) and you have assessed those risks and take full responsibility for any and all such risks associated with your child's participation in the Program/Experience. Cronulla Sharks Football Club and its employees, agents or contractors shall not be liable for any loss, damage, injury or harm suffered by you or your child in connection with the Program/Experience howsoever caused.
You hereby release Cronulla Sharks Football Club and its employees, agents or contractors from any actions, claims or demands which you or your child may have in connection with any such injury, loss or damage (including but not limited to direct, indirect and consequential loss and damage) arising from or in connection with your child's participation in the Program/Experience. You have assessed the activities your child will be involved in during the Program/Experience when deciding whether additional health insurance cover, above that of Medicare, is required and warrant that you have adequate private health insurance cover in place should your child suffer any injury during the Program/Experience.
You consent to medical treatment being administered to your child during an emergency and indemnify Cronulla Sharks Football Club and its employees, agents or contractors against any costs that may be incurred in seeking or administering such medical assistance to your child.
We require at least 14 days notice to be given for a refund for cancellation. If you cancel within 14 days of the Program/Experience, or do not attend a booked Program/Experience, a refund will not be paid. In a case of significant injury or illness, a transfer to an alternative Program/Experience will be offered in which the participant may choose any future Programs/Experiences within a 12 month period to attend.
Whilst we endeavour to have all players attend Programs/Experiences as advertised, player appearances may be subject to change without notice due to the nature of the training schedule, sickness or injury. In which case of a change, we will endeavour to swap the player to the best of our ability.
You consent to the Cronulla Sharks Football Club using your child's name or image (including photograph) in any form or medium for general marketing and promotional activities, unless explicitly advised otherwise in writing.
You agree to receive information, communications, or marketing materials from the Cronulla Sharks related to similar Programs/Experiences, Membership and Ticketing Offers etc, unless explicitly advised otherwise in writing.
